I got this from a USA today article Team Western Kentucky Tornado relief fund (Put this on the memo if you mail a check) Make check to: Kentucky State Treasurer Send to: Public Protection Cabinet 500 Mero St. 218 NC Frankfort, Ky 40601 You can google this and pay online also.
