Help me I.D. this old Stevens 12 gauge.

It looks pretty old and has some unusual features.  It's a hammer single shot break action.  There is a button on the left side to open the action.  The trigger guard is unnecessarily ornate, having a curly section that extends behind the useful portion of the guard - it almost looks like it wanted to be a lever action.  The chamber portion of the barrel is octagon.

What is it?
