Gout Slipped Right In

And bit me hard even though i thought I had been on a reasonably balanced diet with plenty of veggies and fruits.   Either recent medication changes or my sudden propensity for anything sweet did it.

 

Okay I see the light but am a newcomer to really severe attacks that got my attention quick like.   In the process of refining my diet according to John Hopkins, Mayo, and WebMd but I'm a little unsure of how to sort out the more subtle culprits.   For instance,  if I were to pig out and something with high fructose corn sugar or fatty brisket is it gonna manifest itself in pain/swelling within what time period?  Immediately, a few hours, tomorrow?   This is something  not readily apparent in searching google and was hoping there might be a few here with some experience.

Push room temperature water.   Try 4 to 6 quarts of water per day, lots of salad, turkey lunch meat, tomato, celery, etc.  But lots of water.  Get four cases of water and keep track of what you drink.  You’ll notice the difference right away.

After lots of testing and talking, my current doc's opinion is that diet has next to nothing to do with timing or intensity of gout attacks.  I've been on my current meds for the last 8 years, with no attacks and no bad side effects.  I agree with the advice about lots of water; I still avoid organ meats.  So far, so good.

Drink more water. 
 

Myself, I cut out all fish / seafood and haven’t had a full blown flare up in 3 years. Fish was my gout trigger.

Pat, say you ate snapper at lunch then how long before you noticed you'd screwed up?

The next morning for sure...maybe be in the middle of the night. I would say 10-12 hours and then that toothache feeling kicks in.

My Dr put me on Alpurinol . And it pretty much took care of it . He did some blood tests and apparently my body either produces too much or doesn’t get rid of enough uric acid . If they didn’t look into it , it may be worth asking about

The nitrates added to the meat and meat products that you get from the grocery store can trigger major attacks.

Wild game does not have nitrates and my friends (who suffer more than I do) all say that they do not have attacks when eating venison.

DIET HAS A HUGE IMPACT on Gout.  When you’re having problems stay on that bland diet.  Later when you consume a very large meal of all the wrong things. You can feel gout starting in the joints ant the next morning you’ll be in a lot of pain.

 

ALSO, wine, especially red wine, will cause an attack of yout when you are susceptible.  If you make it a habit to consume significant quantities of water, you likely will have few if any problems with gout.   A good Rheumatologist can get a handle on an out of control attack of gout.

I had three bouts of gout 3-5 years ago.

Diet and genetics for me.

I cut down on the proteins.  I don't imbibe but did reduce corn syrup.

Cherries worked for.  Can be fresh or dried but not cooked.

Two schools of thought, diet and medicine. I take a pill and rarely have an episode. It usually comes in my left big toe, but can get into my hands at times and it varies in intensity.

I’ve done both , I also take the same pill to prevent kidney stones . I thought gout was painful until I had those 

cutting beer out was the best thing I ever did , also venison for red meat and more poultry, fish

For me its red wine and seafood that does it.  I asked the pharmacist what is the best over the counter thing to take for gout.  After going through my medical history and what MEDs I'm on, he recommended over the counter Naproxen twice a day for FOUR days only if you are over 65.. He said Naproxen was prescription quality, but it can be obtained over the counter.  It reduced the swelling and the pain by about 90% in about two days.  He also said to stay off the foot as much as possible until it totally disappears.  That's not hard to do now that we are in isolation.

 I'm 6'1" and weigh 200 lbs which is down from 220 I weighed three months ago and drink  like 8 huge t umblers of water or more a day .  I don't eat anything with nitrates or corn sugar and I had to quit booze altogether two months ago.  I eat what I consider is a generous amount of veggies and fruit.   The pill sounds inviting but one of the interactions is with water pills which i take.   I gotta see a doc corona or no corona.....gloves and mask for sure.

Howdy,

I decided I needed real cream in my coffee.

After a couple months first round of gout, real bad.

Doc gave me prescription with several pills tapering down

to one. Cant remember what but druggist said I was lucky to

have avoided gout so long in life.  

I can have a little cheese or milk but not much.

A little delicious port wine cheese and the next morning I hurt.

Worst thing is insisting I want a hamburger not a cheeseburger.

Some stores are dead set on selling cheeze.

I guess it all adds up.

Thanks for that and yeah I need to stay off my feet.....hard not to keep hobbling to the shop.

That “pill” may have been prednisone, or some other steroid or anti-inflammatory.  Long term use can suppress the immune system and create a myriad of other issues.

 

Be careful with that and “Alpurinol” and other gout medications.  You should monitor this stuff yourself and not stay on it long term.  Quiz your MD.  That’s my opinion, anyway.

Last time I was laid up with gout, I was at our summer cottage on Cape Cod, sitting across the table from my son, who was about 13 or so at the time - all nervous energy.  He got excited about something and swung his foot under the table - and scored a direct hit on my gout-infested big toe.  There was a split second of intense pain, and then....total relief.  It felt as though all of those crystals in the toe joint were smashed, and no longer causing the typical gout joint pain.  

 

Now if I could just figure how to duplicate that kind of contact reliably.....

Loophole,  I had a major gout episode in 2016. It was in my right ankle. That was the first and only time I have had gout in my ankle. It’s usually in one of my big toes. 
Anyway, my ankle was very sore but I went to work anyway. Walking was painful and my ankle was stiff. I had to go out to the rail-yard to look at something. As I was stepping across the track I bumped my foot against the rail and it felt like someone had shot me in the ankle. The pain was incredible. I saw stars. 
All of a sudden 3 of my guys come running out of the shop. One of them asks me “Was that you?”

It suddenly dawns on me that my ankle hurts a lot less and I can walk better. 
“Was that me what?”

”That scream. Was that you?”:lol:

I do not recall screaming but I am sure I did. That “bump” hurt like hell! But the pain soon subsided and the swelling went down by the next day. 
I guess, like you, that impact did something to the crystals in my ankle. 
 

Would I bump my foot that way again should I get gout in my ankle in the hopes of easing the pain?

I cannot really say one way or the other, but I am leaning towards not only no, but hell no!

I managed to kick a stool in the shop two days ago and felt anything but relief....still paying for it.  Felt like being smashed with a shop hammer.

Everything has side effects and yes it's basically up to the patient to educate themselves.  However, I have to have some relief albeit temporary if thats the case and was put on a short regime of prednisone today.  I have an excellent infectious disease specialist and we have a good rapport.  So far the three powerful, last ditch antibiotics I'm on don't show any signs of killing my kidneys but they monitor this very closely.
