In one of Chuck Yeager's books he mentions a problem with some fighter jets that were crashing and killing their pilot after an inverted roll. Chuck flew one of these planes to see what was going on, but was wise to wait until he was at several thousand feet to try it. Sure enough, when he rolled the plane it got stuck inverted, and he had to pull back on the stick and loop out of it to recover. At the hanger they pulled the plane apart, and discovered that a bolt and nut were installed from the wrong direction causing the aileron to catch on it. They tracked it down to a factory worker who adamantly insisted that the drawings were wrong and that bolts were always supposed to go in from the top, not underneath. Chuck said he later regretted not telling that bozo how many pilots he got killed.
