I've got one who's second barrel drops off the shear, occasionally, after the first barrel is fired. I shoot light loads anyway so I don't think that's the problem. Suggest you have a gunsmith fix the sear engagement, the symptom might be the result of wear. And might progress to the point of accidental discharges.
