I have a period Model 12 with a "U. S." and flaming bomb prominently stamped on the receiver.... and although it is indeed a Model 12, it has a Model 25-type magazine cap and barrel band arrangement. But there's something distinctive about the mag cap and barrel band on this one that I just can't put my finger on (like an Ithaca)... and the flat-sided bolt....
